Trouble with focus, follow-through, and organization has many possible causes. Chronic sleep loss, anxiety, depression, thyroid problems, trauma, and substance use all produce attention problems that look similar from the outside. So does a genuine attention-deficit/hyperactivity disorder.
Because the causes overlap and the treatments differ, we approach attention concerns as a diagnostic question first. The goal of a first appointment is an accurate explanation of what is happening — not a predetermined outcome.
A thorough psychiatric evaluation is the starting point. We take a developmental history, since attention conditions begin in childhood and a symptom pattern that first appeared in adulthood usually points elsewhere. We review school and work history, sleep, medical conditions, current medications, substance use, and mood and anxiety symptoms, and we ask about collateral information — report cards, a partner’s or parent’s observations — which is often more revealing than self-report alone.
Where the picture calls for formal psychological or neuropsychological testing, we will say so and help arrange the referral rather than proceeding on an uncertain diagnosis.
When the assessment identifies anxiety, depression, a sleep disorder, or a medical contributor, we treat that condition directly. Patients are frequently surprised at how much attention improves once the actual driver is addressed.
Counseling focuses on the practical layer: externalizing memory into systems, breaking work into intervals that hold, reducing environmental friction, and managing the self-criticism that tends to accumulate after years of falling short of one’s own expectations.
Any medication decision is made carefully, discussed fully, and reviewed on a defined schedule. We coordinate with primary care and with school or workplace supports where that helps, and we are candid with patients about what we do and do not prescribe so no one is left waiting on an assumption.
